Was ist die Ursache?

Die Rotatorenmanschette besteht aus mehreren Sehnen, die das Schultergelenk stabilisieren und bewegen. Reißt eine oder mehrere dieser Sehnen, spricht man von einer Rotatorenmanschettenruptur.

Die Ursachen sind unterschiedlich:

Akute Verletzungen, zum Beispiel durch einen Sturz oder Unfall,
Überlastung, vor allem bei wiederholten Bewegungen über Kopf (z. B. bei Sport oder körperlicher Arbeit),
häufig ist der Grund jedoch ein langsamer, altersbedingter Verschleiß. Mit der Zeit wird das Sehnengewebe anfälliger und kann bei Belastung einreißen – auch ohne konkretes Trauma.

Welche Beschwerden sind typisch?

Die Beschwerden hängen davon ab, wie stark das Ausmaß der Rotatorenmanschettenruptur ist. Typisch sind:

Schmerzen bei Armbewegungen, besonders beim Heben oder Drehen,
Kraftverlust oder ein Gefühl der Schwäche im betroffenen Arm.
eine eingeschränkte Beweglichkeit, vor allem bei Überkopfbewegungen,
manchmal auch Nachtschmerzen, die das Liegen auf der Schulter erschweren

Wie wird die Diagnose gestellt?

Zunächst erfolgt ein ausführliches Gespräch (Anamnese), gefolgt von einer körperlichen Untersuchung. Dabei werden gezielte Tests durchgeführt, die bei einer Sehnenverletzung typische Schmerzen auslösen oder Kraftverlust zeigen.

Zur Sicherung der Diagnose sind bildgebende Verfahren wichtig:

Ultraschall (Sonographie): Eine schnelle, strahlenfreie Untersuchung, die bereits Hinweise auf Risse oder strukturelle Schäden geben kann.
MRT (Kernspintomografie): Zeigt das genaue Ausmaß der Ruptur und den Zustand der Sehne. Diese ist u.a. auch wichtig zur möglichen OP-Planung oder bei komplexeren Fällen einer Rotatorenmanschettenruptur.

Welche Behandlungsmöglichkeiten gibt es?

Die Wahl der Behandlung hängt von mehreren Faktoren ab: Größe des Risses, Beschwerden, Alter und Aktivitätsniveau der Patientin oder des Patienten.

Konservative Therapie (also ohne Operation) ist bei kleineren oder verschleißbedingten Rissen oft erfolgreich:

Physiotherapie, um die umliegende Muskulatur zu stärken und die Schulter zu stabilisieren,
schmerz- und entzündungshemmende Medikamente,
gezielte Bewegungsübungen, um die Funktion der Schulter zu verbessern.

Operative Therapie kommt infrage, wenn:

die Sehne stark oder komplett gerissen ist,
die Beschwerden und Kraftschwäche trotz konservativer Therapie bestehen bleiben,
die Schulterfunktion stark eingeschränkt ist.

In den meisten Fällen erfolgt der Eingriff minimal-invasiv mittels Arthroskopie (Gelenkspiegelung), bei der die Sehne(n) mit Fäden refixiert wird (werden).

Was ist die Ursache?

Die Rotatorenmanschette besteht aus mehreren Sehnen, die das Schultergelenk stabilisieren und bewegen. Reißt eine oder mehrere dieser Sehnen, spricht man von einer Rotatorenmanschettenruptur.

Die Ursachen sind unterschiedlich:

Akute Verletzungen, zum Beispiel durch einen Sturz oder Unfall,
Überlastung, vor allem bei wiederholten Bewegungen über Kopf (z. B. bei Sport oder körperlicher Arbeit),
häufig ist der Grund jedoch ein langsamer, altersbedingter Verschleiß. Mit der Zeit wird das Sehnengewebe anfälliger und kann bei Belastung einreißen – auch ohne konkretes Trauma.

Welche Beschwerden sind typisch?

Die Beschwerden hängen davon ab, wie stark das Ausmaß der Rotatorenmanschettenruptur ist. Typisch sind:

Schmerzen bei Armbewegungen, besonders beim Heben oder Drehen,
Kraftverlust oder ein Gefühl der Schwäche im betroffenen Arm.
eine eingeschränkte Beweglichkeit, vor allem bei Überkopfbewegungen,
manchmal auch Nachtschmerzen, die das Liegen auf der Schulter erschweren

Wie wird die Diagnose gestellt?

Zur Sicherung der Diagnose sind bildgebende Verfahren wichtig:

Ultraschall (Sonographie): Eine schnelle, strahlenfreie Untersuchung, die bereits Hinweise auf Risse oder strukturelle Schäden geben kann.
MRT (Kernspintomografie): Zeigt das genaue Ausmaß der Ruptur und den Zustand der Sehne. Diese ist u.a. auch wichtig zur möglichen OP-Planung oder bei komplexeren Fällen einer Rotatorenmanschettenruptur.

Welche Behandlungsmöglichkeiten gibt es?

Die Wahl der Behandlung hängt von mehreren Faktoren ab: Größe des Risses, Beschwerden, Alter und Aktivitätsniveau der Patientin oder des Patienten.

Konservative Therapie (also ohne Operation) ist bei kleineren oder verschleißbedingten Rissen oft erfolgreich:

Physiotherapie, um die umliegende Muskulatur zu stärken und die Schulter zu stabilisieren,
schmerz- und entzündungshemmende Medikamente,
gezielte Bewegungsübungen, um die Funktion der Schulter zu verbessern.

Operative Therapie kommt infrage, wenn:

die Sehne stark oder komplett gerissen ist,
die Beschwerden und Kraftschwäche trotz konservativer Therapie bestehen bleiben,

In den meisten Fällen erfolgt der Eingriff minimal-invasiv mittels Arthroskopie (Gelenkspiegelung), bei der die Sehne(n) mit Fäden refixiert wird (werden).
